Integrated Care Strategy
The strategy has now been signed off and I’ve attached the final glossy version. Attention now focuses on implementation and in particular the three life stage themes of “Live Well, Stay Well, Age/Die Well”. We’ll be providing opportunities to explore VCSE engagement in these themes at the June 26th Event, “Moving Forward Together”.
Moving Forward Together
Please find attached some early details of the “Moving Forward Together” event on 26th June. It will be the first time that the VCSE sector and partners across the whole system will have an opportunity to meet face to face. Please share with anyone you think might be interested. ICB Joint Forward Plan
The Derby and Derbyshire NHS system has to develop a Joint Forward Plan for the next 3-5 years and is asking for input from partners to help shape this. A cross sector group is working on this and has asked for some feedback by Friday 12th May. The timescales are very short and we’ve been assured that this will be an iterative document with chance to change after this point. I’ve attached two documents. The first is some summary background information and the second is a SWOT template for partners to fill in. Kim Harper from Community Action Derby sits on this group but because of the tight deadlines, if you would like to contribute could you please send the completed SWOT to samuel.kabiswa1@nhs.net by Friday 12th and copy in Kim at kim.harper@communityactionderby.org.uk and me.
There is a good diagram on the accompanying information that shows how all these plans and strategies fit together.
TeamUp
A big thank you to those who were able to come to the recent TeamUp session. TeamUp is the initiative to try to keep people in their own homes and out of hospital and will be key to the Age Well/Die Well theme in the Integrated Care Strategy. It was a really good session and has given the TeamUp leads much food for thought. It feels like the start of a dialogue and I’ve attached the notes from the meeting.
VCSE Representatives on Integrated Care System structures
We had our first meeting of VCSE representatives involved in the different parts of the Integrated Care System. We currently have 19 representatives filling 24 spaces on 14 different partnership structures (and many other time limited, informal, developing ones). The meeting was to explore how the representatives link with each other and with the wider VCSE constituency. We’ll be meeting regularly and I’ve attached the notes from this first session.
End of Life Care
It is vitally important that VCSE organisations are involved at the earliest possible opportunity in design and development. The End of Life Care Board has strong involvement from hospices and there may be a range of other organisations that feel they are able to contribute and shape this work. Memorandum of Understanding
I’ve redrafted the MoU following feedback from VCSE and system colleagues (see attached). This hasn’t always been easy as there isn’t always consensus as to content! If you’ve any comments on this version, please let me know by Thursday 11th May. Our VCSE Alliance meeting on 23rd May at 11am will discuss and sign this off and then the progress is,
25th May approved by the Integrated Place Executive
7th June signed off by Integrated Care Partnership and recommended to Integrated Care Board
15th June signed off by Integrated Care Board
26th June launched at system wide VCSE event.
