Opportunity Description: This service provides support to people who have recently been discharged from hospital in Nottinghamshire and have little or no support system. Volunteers are paired with someone for up to 6 weeks and will offer support by doing safe and well calls and visits to ensure the person is comfortable in their home. Volunteers will feedback information to our support coordinators who will put support in place for this person.

Who we are looking for: We are looking for kind and compassionate people who are wanting to make a difference in their community.
Requirements: Will require a Criminal Record Check (DBS), Driving License, Own vehicle or have access to a vehicle

Practical considerations: Support coordinators, based in the hospital, will match volunteers to people based on their location and availability. Legitimate volunteer expenses are paid in all roles on a monthly basis in line with HMRC guidelines. A small amount of paperwork needs to be submitted on a monthly basis and volunteers are provided with the necessary documents to support this.
